Our care

Care that starts today, and stays.

Same-day care

When you need to be seen, you're seen — same-day care from board-certified physicians, with the local access a small town deserves.

Chronic disease management

Ongoing care for chronic conditions through the US-LTN program: daily health monitoring, regular check-ins, and a local clinical team that knows you.

Lifestyle medicine

Plain-language guidance on nutrition, movement, and daily habits — practical steps that help patients manage, and often improve, chronic disease.

Part of the US-LTN network

The network's Connecticut hub.

Litchfield Health anchors the Connecticut operations of US-LTN — the United States Local Triage Network — and serves as the network's northern clinical hub. US-LTN is a program of Rappahannock Health Corporation, a 501(c)(3) non-profit organization (EIN 86-3528712), so the care we deliver serves a charitable mission, not private profit.

Litchfield Health is also a pilot site for the CMS ACCESS model, part of the network's work to bring daily health monitoring and lifestyle medicine to the communities that need them most.

US-LTN — the United States Local Triage Network
